Your first appointment is an opportunity for us to get to know you and understand your health goals.

During your visit, we will:

  • Review your medical history and current concerns

  • Discuss preventive care and recommended screenings

  • Review medications and allergies

  • Develop a personalized care plan

Please plan to arrive 15 minutes early to allow time for check-in.

What to Bring

To help your visit go smoothly, please bring:

  • A valid photo ID

  • Your insurance card (if applicable)

  • A list of current medications and dosages

  • Relevant medical records or test results

  • Completed new patient forms (if available)

    Same-day appointments may be available for acute concerns.

    If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room.


  • Secure telehealth appointments are available for select conditions and follow-up visits. Telehealth is not appropriate for emergencies or all medical concerns.


  • We are still in the contracting process, but we plan to accept most major insurance plans, including Medicare. Coverage varies by plan.

    **We do not accept Medicaid.

    If you have questions about insurance, billing, or self-pay options, our front desk team is happy to help.
